We reserve the right to revise or update this notice at any time. Updated versions will be made available at our pharmacy and on our website.

We may use and disclose your health information to provide, coordinate, or manage your healthcare and related services. This may include communication with physicians, healthcare providers, hospitals, caregivers, and other professionals involved in your care.

We may use and disclose your information to bill and collect payment from insurance companies, pharmacy benefit managers, Medicare, Medicaid, or other third parties responsible for payment of your healthcare services.

Text Messaging (Nextiva)
We may send text messages related to your pharmacy care and services, including:

  • Prescription readiness notifications

  • Refill reminders

  • Delivery coordination

  • Pharmacy follow-up communication

  • Customer support responses

  • General pharmacy service updates

By providing your mobile phone number to Bald Mountain Pharmacy, you consent to receive SMS text messages from us related to your pharmacy services.
Message frequency may vary depending on your pharmacy activity and communication preferences. Message and data rates may apply depending on your mobile carrier plan.
You may opt out of receiving text messages at any time by replying STOP to any message. For assistance, reply HELP or contact us at 248-607-0205.
Bald Mountain Pharmacy does not sell, rent, or share your mobile phone number or SMS consent information with third parties for marketing purposes.
While Bald Mountain Pharmacy uses HIPAA-conscious communication practices and secure systems where applicable, standard SMS text messaging may not always be fully encrypted. Please avoid sending highly sensitive medical information through text messaging.
Pharmacy Messaging Systems (PrimeRx)
We may use pharmacy software and communication systems, including PrimeRx and related pharmacy technologies, to provide:

  • Prescription notifications

  • Refill reminders

  • Medication adherence communication

  • Pickup reminders

  • Pharmacy service updates

Email Communication
We may send limited email communications regarding pharmacy services, prescription information, refill coordination, or customer service inquiries.
SMS Consent and Website Forms
When submitting forms through our website, including prescription transfer requests, refill requests, contact forms, or other communication forms, you may be asked to provide consent to receive SMS communications from Bald Mountain Pharmacy.
By voluntarily providing your phone number and agreeing to receive communications, you consent to receive pharmacy-related text messages from Bald Mountain Pharmacy. Consent is not a condition of purchasing products or services.
Patients may withdraw consent at any time by replying STOP to any text message or by contacting the pharmacy directly.
Uses That Require Your Authorization
We will not use or disclose your information without your written authorization for:

  • Marketing purposes unrelated to your care

  • Sale of protected health information

  • Most uses of psychotherapy notes

  • Any purpose not described in this notice unless otherwise permitted or required by law

You may revoke your authorization at any time in writing, except to the extent action has already been taken in reliance upon it.
Your Rights
You have the right to:

  • Access and obtain copies of your medical records

  • Request corrections or amendments to your records

  • Request a list of disclosures

  • Request restrictions on certain uses or disclosures

  • Request confidential methods of communication

  • Receive a paper or electronic copy of this notice

  • Be notified following a breach of unsecured protected health information

Requests may be submitted in writing to the pharmacy.
Data Retention and Security
We maintain administrative, physical, and technical safeguards designed to protect your information.
Records are retained in accordance with federal and Michigan record retention laws. Some information may remain in secure backup systems for a limited period as required for legal, operational, or disaster recovery purposes.
Michigan-Specific Privacy Protections
Michigan law may provide additional protections regarding prescription information, controlled substance records, mental health records, communicable disease information, and other sensitive healthcare information.
Bald Mountain Pharmacy complies with applicable Michigan privacy laws in addition to federal HIPAA requirements.
Third-Party Service Providers
Bald Mountain Pharmacy may utilize third-party vendors and service providers to assist with pharmacy operations, communication systems, website hosting, prescription processing, payment processing, and technology support.
These providers are required to maintain appropriate safeguards and confidentiality protections consistent with applicable laws and regulations.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with Bald Mountain Pharmacy or with the U.S. Department of Health and Human Services Office for Civil Rights.
You will not be retaliated against for filing a complaint.
Protection of Personal Information 
No mobile or messaging consent information will be shared with third parties/affiliates for marketing/promotional purposes.
